Summary: This position is responsible for performing all new client comprehensive assessments and referring these clients to any and all appropriate Sweetser services. ESSENTIAL FUNCTIONS:
Completes client assessments as scheduled by the Promise Line.
Responsible for all assessment documentation to include: diagnosis, assessment, mental status, level of care, progress notes, releases etc within the ElectronicHealth Record (EHR).
Submits referrals for immediate scheduling within identified standards with designated service(s).
Perform concurrent documentation.
Minimum of twelve (12) hours per week
Masters Degree in a related field (Psychology, Human Development, Social Work, etc)
CERTIFICATES, LICENSES, REGISTRATIONS:
LCSW, Independent Licensure required
Maintain a valid state drivers license.
Experience with assessment of adults, children and residential.
Persons with lived experience in the Mental Health and/or Substance Abuse system are encouraged to apply.
KNOWLEDGE AND SKILLS:
Knowledge of Medications, crisis assessment skills and familiarity with community resources.
Computer skills, including Electronic Health Records (EHR)
Customer service skills
Excellent communication skills
